China Is Outpacing the U.S. in Clinical Trials. The FDA Just Responded.

A Growing Gap

For decades, the United States has led the world in clinical research. That lead is shrinking. According to a June 2026 report by the Cure Innovation Index, China's share of global clinical trial starts has grown from 1% in 2009 to 32% in 2025, nearly matching the United States' 35% share. Early-stage trials in China are estimated to run 50 to 70% faster than in the U.S., and Phase 1 trials cost roughly 50 to 60% less. This shift did not happen by accident. China's government has systematically invested in its pharmaceutical and research infrastructure through state-directed initiatives like Made in China 2025, building domestic capacity across the entire drug development pipeline.

Structural Barriers

The reasons the U.S. is falling behind are structural. Starting a clinical trial in the U.S. can take years. In Australia, a comparable trial can begin within 70 days of a final protocol submission. Meanwhile, China's dual-track regulatory system allows experimental therapies to be tested through investigator-initiated trials with far less red tape. Since 2018, these trials for cell and gene therapy products have enrolled over 30,000 participants. As more trials move overseas, American patients lose access to cutting-edge treatments being tested abroad, and the U.S. loses its foothold as the center of global biomedical innovation.

The FDA Responds

The FDA is now responding. In June 2026, it launched a new initiative to reduce clinical trial development timelines and reverse the growing trend of research moving to overseas sites. Bipartisan support exists in Congress for broader FDA reform. Whether these efforts will be enough to close the gap remains to be seen, but the urgency is clear. The race to develop the next generation of medical treatments is no longer exclusively American.
